Your health insurance may cover the entire cost for your nutrition counseling. We can look into your benefits for you, or you can check with your insurance provider. Ultimately, it is your responsibility to know and understand their own benefits.

If you have a flex spending account through your employer we can accept that for payment of co-pays, deductibles and all private pay options.

If your insurance provider does not cover nutrition counseling or if you prefer to pay out of pocket, we offer the following option:

There will be a 10% discount for seniors on the package.
*5 visit packages must be paid for in full at the time of purchase. There is no expiration date on the visits and they can be used at any time. There is no refund for partially unused packages. If a patient does not want to use their remaining visits for whatever reason they may transfer them to a friend or family member. Refunding a full, unused package is determined on a case by case basis by the Nutrition Director.
In effort to improve the payment process, and make your experience with us more convenient, Evolution Nutrition will ask you to provide a credit or debit card for your file. If you cancel or reschedule your appointment with less than 24-hour notice you will be charged a $25 cancellation fee.
